About Givens Choice

Givens Choice, a Continuing Care at Home program, is designed for those who hope to remain in their own home as they age. It brings the security of a Life Plan Community to you, with a plan that supports your ability to stay in the home you love, protects your financial assets, and removes the burden of navigating it alone.

Care Management

A strong partnership is the cornerstone of Givens Choice. From the start, your Care Manager gets to know you, your preferences, and goals for aging in place. As your partner, they help you navigate decisions with confidence and clarity, while always supporting your autonomy and independence.

Care Coverage

The right support at the right time makes all the difference. Our goal is to implement supports proactively rather than react to a crisis. Care Managers coordinate care by aligning providers, services, and supports around each member’s needs and goals.

Financial Protection

A Givens Choice membership replaces uncertainty with a plan, one that makes the cost of non-medical long-term care more predictable while helping preserve your assets.

Membership

Eligible applicants who enroll in Givens Choice pay a one-time membership fee and monthly maintenance fees which vary based on your age at enrollment.
