As needs change, your Care Manager helps identify, arrange and oversee appropriate services. Instead of navigating providers, decisions and logistics alone, you have one trusted point of contact who understands your preferences and keeps your goals at the center of the plan.

Your Care Manager

Your Care Manager is the connection among you, your family, your providers and the services available through Givens Choice. By getting to know you before support is needed, they can offer guidance that is proactive, personal and grounded in what matters most to you.

Your Care Manager Can:

Help you anticipate and plan for changing needs
Offer guidance when questions or concerns arise
Help you understand available options
Coordinate providers, services and transitions
Work alongside family members and other professionals
Monitor services and adjust the plan as needed

Planning & Prevention

Membership includes resources that help you plan ahead, maintain independence and respond to changes early.

Financial Protection

The rising cost of long-term care can be difficult to anticipate and manage. Givens Choice takes a proactive approach, putting the right support in place at the right time with the goal of maintaining independence and, when possible, delaying or avoiding more expensive levels of care. Each membership plan defines the daily amount Givens Choice will pay toward eligible long-term care services and the portion, if any, paid by the member, allowing you to choose the level of financial protection and predictability that best fits your plan.
