Relationships are the heart of Givens Choice. Our Care Navigators work with members from the beginning to establish a relationship built on trust. By getting to know you, your home, and your vision for the future, they can coordinate care that is proactive and focused on your unique needs instead of reactively in a crisis.
If you need support, your Navigator will do more than provide a list of resources. They will take the lead in putting services in place, coordinating the scheduling, care planning, and payment of any long-term care services you need. Your Givens Choice membership includes the range of long-term services and supports including:
Home Care Services: When you have in decline in just one Activity of Daily Living, we will put support in place to maximize your independence. This support will be tailored to your unique needs and can help with medication reminders and light housekeeping all the way to bathing and dressing.
Assisted Living and Memory Care: Whether it be at Givens Estates or another local facility, your Navigator will help you choose an assisted living that provides the right mix of services and amenities to meet your needs and maximize independence.
Skilled Nursing & Rehabilitation: If you need skilled, around the clock nursing care for a short or long-term stay, we will coordinate the logistics, so you don’t have to.
Your membership also includes a home bi-annual home safety assessment conducted by a Certified Aging in Place Specialist, transportation for medically necessary appointments, 24-hour emergency response system, and a referral service for qualified professionals when you need home maintenance, legal services, medical equipment, etc.
As a Givens Choice member, you are encouraged to take advantage of the amenities offered at our Life Plan Communities, Givens Estates and Givens Highland Farms.
