Givens Choice has 3 membership plan options that allow you to choose the plan that best fits your needs, your insurance, and your budget.

Platinum Plan Members receive 100% coverage of all Givens Choice services with the most comprehensive rate.

Gold Plan Members receive 100% coverage of Care Navigation and assessment services and have a 20% copay for long-term services and supports.

Silver Plan Members receive 100% coverage of Care Navigation and assessment services and have a 50% copay for long-term services and supports.

Although the pricing of each plan varies, each plan has a per-person membership fee and per-person monthly fee.
