As a member of Givens Choice, you will have the peace of mind that comes from knowing you can remain in your home with a secure plan for care, that your financial assets are protected from the rising cost of long-term care, and most importantly, that you and your loved ones will not feel the burden of planning and managing your long-term care needs.
Relationships are the cornerstone of Givens Choice. A Care Navigator will work with you from the beginning to get to know you personally and understand your vision for aging in place. Throughout your lifetime they will ensure that your care needs are met by coordinating a full continuum of services such as home care, assisted living, or nursing home care. You and your family will benefit from the expertise and guidance of your care navigator who will support you through any change in your health. The right care for you, at the right time is just a phone call away.
Along with personalized navigation and coordination of services, a Givens Choice membership assures that your non-medical long-term care costs are both covered and contained. Your Givens Choice membership fees will never be increased because of a change in your care needs. If you require a move to assisted living or nursing home care, your Givens Choice membership provides access to that level of care and financial coverage, protecting you against potential care costs that could surpass $100,000 per person per year on average.
Eligible applicants who enroll in Givens Choice pay a one-time membership fee and monthly maintenance fees which vary based on your age at enrollment. Members choose from 3 membership plan options to meet your personal financial objectives.
For example, members who choose the Platinum Plan receive 100% coverage for all Givens Choice services including care coordination, home care, assisted living, memory care, and nursing home care. For members who have long-term care insurance, our Gold and Silver plans (80% coverage and 50% coverage) offers the potential to add one’s existing insurance with our program at a reduced cost.”
